Thanks very much for supporting My Name, My Face.

We are now going to ask you to enter your name below and upload a photographic portrait of your face, to lend your name and your face to the campaign.

• Step 1: Please enter your name

• Step 2 (for high profile individual): If you are a high profile individual, please provide the name and email address of your agent or manager, so that we can contact them to seek verification of your support for My Name, My Face (and please tell them to expect us to contact them for that purpose).

• Step 2 (for non high profile individual): If you are not a high profile individual, please provide your email address. We will use this as part of our process to verify that you are who you say you are. You will receive an email from mynamemyface@mills-reeve.com to confirm that your support has been registered, and that your name and face will be added to the My Name, My Face supporter gallery once the identity verification process is complete. That email will also include a secure link which you can use to tell us if you change your mind about supporting the My Name, My Face campaign.

• Step 3 (optional): Please provide details of your occupation if you are comfortable doing so. My Name, My Face is not just about famous people. Everyone deserves the protection which the campaign promotes. Your name, face and occupation will be added to our gallery to encourage everyone to support the campaign, whoever they are and whatever they do.

• Step 4: Please upload a photograph of your face (or just take and upload a selfie). Please provide a head and shoulders shot only, and try to make sure the background is plain.
